If your book needs to be shipped, include your shipping address and double check it before submitting. The author is not responsible for lost shipments if the wrong address was provided. **US shipping only.** Make sure to hit SUBMIT.
Then, click the PAY button and select either a local pick-up ($30) or shipping ($35). Choose your payment method and complete the order.
You must submit BOTH the form and the payment so I have all of the information needed to complete your order and reserve a copy from the first print run.
